Overview

RN MDS/Care Plan Coordinator Supervision and Direction. The MDS/Care Plan Coordinator receives general supervision from the Director of Nursing Services. The MDS/Care Plan Coordinator will comply with established policies and procedures of the facility. In the absence of the Director of Nursing, the MDS/Care Plan Coordinator will accept advice and instruction from the Assistant Administrator, Administrator, or Executive Director or, in their absence, the Regional Director.

Educational/Licensing Requirements:

  • This position requires the candidate be a graduate of an accredited school of registered nursing and licensed in good standing. Must possess:
  • Current, unencumbered, active license to practice as an RN in the Commonwealth of Virginia, or a compact state participating in the Nurse Licensure Compact.
  • Current CPR certification, or acquire CPR certification within 60 days of hire.
  • The states participating in the Nurse Licensure Compact are: Arizona, Arkansas, Delaware, Idaho, Iowa, Maine, Maryland, Mississippi, Nebraska, New Mexico, North Carolina, North Dakota, South Dakota, Tennessee, Texas, Utah, Virginia, Wisconsin, Kentucky, and Colorado (January 1, 2005 §54.1-3030, et. seq., Code of Virginia).

Key Responsibilities

The RN MDS/Care Plan Coordinator will:

  • Be an RN and be available to work flexible hours.
  • Be responsible to schedule all assessments and set appropriate ARD dates.
  • Be responsible for the completion of the MDS process, including the MDS form, CAAs, Triggers, and formulating an individualized plan of care for each resident following the guidelines established by the State and Federal regulations.
  • Maintain and distribute to the Interdisciplinary Care Plan Team a master schedule of all assessments to ensure appropriate timing.
  • Ensure other disciplines complete their sections of the MDS and care plan.
  • Ensure all signatures are complete on the MDS and Care Plan.
  • Utilizes available software edits to identify errors prior to transmission.
  • Transmit to the State database on a weekly basis or within the designated time frame.
  • Review validation reports for rejections and make necessary corrections on each transmission.
  • Review quality indicators for areas greater than 70%. Upon transmissions, give a copy of the necessary reports to the Director of Nursing and the Administrator.
  • Review resident level summaries for missing assessments and make corrections.
  • Review 24-hour reports, labs, charts, and any other pertinent records on both nursing units daily to evaluate for significant change.
  • Report the Quality Assurance Committee numbers of assessments completed, transmitted, corrections, and those accepted in the State database. Develop a plan of action to improve accuracy and reduce errors for MDS submissions.
  • Assist with in-service training for facility personnel with special emphasis on the RAI process and serve as a resource person.
  • Provide “hands-on” assistance to facility residents as necessary and in emergency situations as needed.
  • Assist in the review and development of nursing policies and procedures.

  • Observe and protect resident rights, including the right to confidentiality, privacy, and dignity.
  • Report signs of resident neglect or abuse immediately.
  • Attend Quality Assurance, Utilization Review, and other meetings as directed.
  • Performs any other related duties that may be assigned.

Qualifications

  • Current Registered Nurse license
  • CPR certification or ability to acquire within 60 days
  • Experience in senior care is preferred

*Sign-on bonuses are offered in incremental payments over a 24-month period for full-time positions only.
